IMLCA Twelve Month Payment Plan

Reminder: You must submit the signed Twelve Month Payment Plan Form along with your 1st payment for acceptance of the plan.
The signed form MUST be received by JANUARY 16, 2019 to be considered on the Payment Plan.
If you have not submitted your signed form, copies were sent with your invoicing, are available in the office, and/ or you may request a form from Tori Rasmussen, Collection Agent via email collections@indianmountainlake.com
